Cataract Direct Referral Scheme

NHS walsall has a direct cataract referral scheme.

 

1.         Referral

 

The following documentation can be found at the end of the text on this page:

 

·        C1 - Direct Referral of Cataracts form is designed to be completed by the optometrist who should offer the patient choice of provider. Completed forms must be sent to the chosen provider and a copy sent to the patients GP.

·        C2 forms - to be sent to the GP who will send any additional information to the provider.

·        Claim forms - to be sent back to the PCT requesting Payment for the attention of Diane Mathews.

·        Ensure that the Patient Consent and Information Exchange Form is completed and signed by the patient.

 

These forms are to be sent to the PCT along with the notification of referral and claim form in order to send off for payment.

 

2.         Providers

 

Patients can choose to attend any of the hospitals below:

           

·        Walsall Hospitals, NHS Trust, Manor Hospital

·        Royal Wolverhampton Hospital

·        Good Hope Hospital

·        Sandwell & West Birmingham Hospital

 

If the patient does not have a Walsall GP you will need to contact the PCT where the patient’s GP is registered to gain access to that PCT’s scheme.

 

3.         Claim Process

 

Please complete the form headed ‘Walsall Teaching PCT – Notification of Referrals for Cataract Surgery’ for each month with all the patient details  requested for each patient referred. Please ensure that you complete the ‘chosen hospital provider’ section and send back to the above address.

 

Attach the Optometrist Claim Form for the total amount of claim stating your practice name and signature of the optometrist.

 

Please return these to Alison Simmons Primary Care Contracts Manager

 

Please note that, all forms are master copies and you should photocopy for use.

 

For anyone who has not sent back their Specimen Signatures / Counter Signatory Form can they please do so ensuring that you state which practice you are from.  If we do not receive your specimen signatures then unfortunately we will not be able to process any claim.

 

Registration for the Cataract Direct Referral Scheme

All new mandatory and additional services contract holders from Jan 2011 onwards are required to be registered to refer directly.  This can be done by contacting Alison Simmons Primary Care Contracts Manager 01922 618348 Alison.Simmons@Walsall.nhs.uk .  No additional training for registration is  required but knowledge of referral guidelines is essential and at least one performer/optometrist at the contractor's practice/business will be  required to provide evidence of observing and/or conducting three direct cataract referrals whilst being mentored/supervised by an optometrist within NHS Walsall previously accredited to refer directly.
