Transition Team Launches New Website

The Health Transition Team at NHS Walsall Community Health has launched a website designed to give young people with physical disabilities help and advice at the touch of a button.

The new website – called Transition Together – was created in partnership with both young service users and health professionals.  It offers easy to use information about how and where to access services covering a wide range of issues including employment, training, training, transport, money matters and health.

Designed specifically for those going through the change from childhood to adulthood, one of the main aims of the site is to provide users with the information they seek in three clicks or less.
