Nominate your Community Health Worker of the Year 2005

Nominations by 14 October

Do you want to nominate a doctor, nurse, or other health worker for an award? The Walsall Community Health Worker of the Year Award is open for nominations for health workers — including administrators and receptionists — working in the Walsall Community. This includes GPs, dentists, optometrists and pharmacists and mental health workers, but not people who work in the acute hospitals. To nominate someone you need to give their name, the reason for your nomination, and your own contact details.

To nominate someone, you can download and post this form, or you can email  us. If you are emailing, please state the name of the person you are nominating, the reason for the nomination, and your contact details if these are different from the email address you are sending from.

The awards will be presented on 27 October.
